If you do not have documentation supporting your qualifying condition, you will need to schedule a visit to have our concierge medical professional confirm your diagnosis, prior to proceeding with the medical marijuana card certification process.

We have an excellent Patient Care Manager, who personally sees to it that everyone is contact within one business day of submitting their appointment request form.

We collect the information on these forms so that you do not have to fill out additional paperwork at the time of your appointment.

You may schedule your appointment at the time and place of your choosing. Your home, a coffee shop, restaurant, or anywhere within safe reason is an acceptable meeting place for our concierge medical professionals.

Please note that the doctor will try to make contact with you on the day of your appointment, prior to arriving at the agreed upon location. If the doctor cannot reach you, your appointment will be rescheduled at no additional charge.

Due to the nature of a concierge service, it is very important for us to be able to make contact with our patients on the day of the appointment, prior to arrival for both safety and legal purposes.

Please make a point to have your mobile device on and near your person at least 30 minutes before your appointment time.

The doctor will review your documentation supporting your qualifying condition and confirm your identification is valid.
Payment is due in full at the time of the appointment.
Upon completion of the evaluation, you will receive an official written certification from our physician for you to upload to the state with your application.

By the time of your appointment, we have already collected nearly all of the information we need.

Once the doctor have reviewed your information and confirmed that you are a good candidate for medical cannabis therapy, you will both sign the official written certification form for you application to the State.

Please note that your full payment is due at the time of the appointment, and you will only receive the written certification form when payment is submitted.

The entire appointment usually takes less than 15 minutes, but we try to allot plenty of time for you to ask the doctor any questions you may have.

To complete an application for medical marijuana, the State of Arkansas requires you to have:

An Official Written Certification Form from a physician,
A Valid Arkansas state ID (Not a federal ID, like a passport)
$50 Application fee only payable by credit card

Once you have received the Official Written Certification Form from our concierge physician, you can go to the Arkansas State website and apply there.

Once you’ve submitted your application, they administer your medical card within 10 business days.

However, several of our patients have reported getting their card back from the State in much less time.
